For allergy injection "templates" I just use an Excel spreadsheet template for each patient. Easy to create and bring up by my nurse when the patient checks in.
As for allergy (practice) templates, that's still a work in progress but send me a message M and you can take a look.

Oops…I misspoke or misposted re allergy shots. I use Excel spreadsheet for allergy VIAL data re composition, strength, schedule etc. imported to each specific patient. I still use paper preprinted instruction sheets with each vial. Of course it would be easy to construct an Excel document and record each injection but I don’t think this would be as efficient.
