I have one printer in our office that refuses to print these forms correctly. The printer is a HP Officejet K5400 and I have tried switching to a new printer (same kind), uninstalling and reinstalling the printer, switching the Print Processor(under Advanced Settings) to several different settings, trying to adjust page height and such, and I cannot figure out why it prints the way it does. It prints like this:
First Page with most of the info

Second Page with the red boxes and very little info

Any suggestions?

Hello Paul,

With the CMS 1500 form you will see little adjustment bars on the top and to the right of the form. You use them to adjust the print out. This can often be a time consuming process, because you can only move them by a hair and then you have to click set print position then print. You have to do this till you get it in the right position. Also to let you know these settings are local to the windows user you are logged in as, and to the computer you are on. So if you log in as a different user on the very same computer you will need to set that print position for that windows user log in as well. Unfortunately this would need to be done for each computer you intend on printing from. Fixed! Thanks for the tips. I didn't think about the alignment because the pages were both aligned in a seemingly correct way. I Adjusted the print position further left and up, then adjusted in small increments until alignment was correct. Thank you for the help!

Medicare keeps returning my 1500 forms saying that it is not the proper form. (Most of the claims go out electronically and I don't have any problem with that). The form looks correct to me.

Is it the paper?

This is just a guess, but I would imagine that their scanners recognize only the page alignment to tell if the form is correct, which would mean if you were off by the width of a character (or check box) that it may register as a bad form.

I tried adjusting on two different printers/computers in my office with V 5 and receive the message that there isn't enough vertical space. The data is about 1/4 inch too high on the 1500 forms. Any suggestions?

The preview screen that pops up seems to be a method of adjustment. Try "scrolling" up or down on the vertical scroll bar and print some test forms on blank paper (use a HCFA form behind it if you need to check alignment) and keep trying small adjustments until it fits. Worked for me.
