I am looking for ideas of where VIS date can be recorded when an immunization is given and where the patient/parent signature is recorded acknowleding receipt of the VIS. Thanks.

Version 5 had a place for VIS on the vaccine entry (when saw at AOA convention) - so it will be there. It even had a link to download most current ones built in. The difficulty is that there is no place for the signature - that is the consent form that somewhat places liability on the government vaccine fund. I still use the vaccine consent/admin form. I then scan them in.
