There has been alot of discussion here concerning the need for AC to have a method to put in reminders for patients. However, there should also be a set place in a patient's chart to check for the date of a previous test. For instance, date of last mammogram or date of last pelvic exam&pap smear. Where do other users of AC place this information? Is there a regular place that you use so that the dr. can always look at the same place in the chart to check for this information? Where could AC add this function?

Would appreciate some suggestions on this one.

I use past history box to put in these:

Past Medical History:
Childhood illnesses:
Past Surgical History:
Tests:
Immunizations:
Advance Directives:

(We did not create past hospitalization line intentionally as there is a overlap of past surgical history and hospitalization and sometimes with past medical history like severe pneumonia).
I want all on one page, I dont want to keep looking into some other place.


One could use the tracked data info- where a file clerk could enter the info. But this will be on a different page.

I also use the PMH. I have a template called "Complete" and either on the first office visit for a patient or at their first Complete exam this template is put into the PMH
MEDICAL
SURGICAL
INTERVENTIONS
Pneumovax:
Tetanus:
Hepatitis:
Zostavax:
PPD:

Colonoscopy:
Mammogram:
PAP:
Bone density:


I can then note the dates easily every time I pull up a patient and see when they are due.

Other things I want to track can also easily be noted, like when the last carotid doppler was done on a patient with a bruit or CXR on a smoker, etc. This has worked really well for me, an Internist.

I use the PMH as well but I would rather have a place in AC designed to track this. I do use the item tracking under the summary page but it is cumbersome. Hopefully in a forthcoming update...

If you have used the "Order"button to order a test, x-ray, or procedure--that data is stored in AC's database, including the order and the date. Would it not be easier to design a small program to retrieve the data out of AC's data base and give it to you as a "Report of Last Time the Following Tests were Ordered" for Mr. So and So. I am sure there are programs that you can buy to search SQL databases and generate reports by whatever parameters you define.

Your thoughts on this solution are welcome

I use the Alerts/Directives section to track prevention issues (i.e. mammogram, colonscopy, carotid and AAA screens etc). I believe I picked this trick up from Bert in a post from 2007 or 2008. It prints up nicely on the summary sheet that way for a patient (a trick I also picked up from someone else's post yet unfortunately I don't remember who).

As far as tracking future tests however, I struggle. I use reminders to staff members to check and make sure we got results of tests ordered or to order a test. I am lucky to currently have a good team, yet this method can be risky if you don't have trustworthy staff (like I've had in the past). I agree, having a dedicated place to track prevention issues and ordered tests would be great.
