Can anybody tell me where these are to be placed on the 1500 forms? I put them in the modifier section and all were returned unpaid. On the web sites it is not clear, they talk about denominators but that doesn't mean anything to me. Thanks.

Seems to me from reading this link http://www.apa.org/practice/pf/feb97/fednotes.html that they would go in the CPT code boxes.

g8443- E-Prescription Generated.
G8445- Office visit but no Prescription generated
G8446- E-Prescribing system available however cannot be sent due to state/federal law or patient asked for script in hand!

Thanks, I have a list of the G-codes for prescribing but I don't know where they go on the 1500 for?

Add them as extra CPT codes on the visit. So say you do an office visit 99214. In the CPT boxes (24d on the 1500 form) you would put 99214 and the other info as needed. Then below that you would put codes for other things, such as pulse oximetry or what-have-you then after the last code, add in an extra code such as G8443 on a new line, just as if it were another procedure.

That I do not know, I have been given feedback that as long as you add the codes to the Superbill and check one that is sufficient but i dont work for Medicare! Sorry i couldnt be more help!
