Can anybody tell me where these are to be placed on the 1500 forms? I put them in the modifier section and all were returned unpaid. On the web sites it is not clear, they talk about denominators but that doesn't mean anything to me. Thanks.

Here is something that was really helpful to me, but may not be what you need. http://www.palmettogba.com/Internet/cms1500.nsf/index.html# Hopefully someone more knowledgeable about billing can give you a direct answer.

Add them as extra CPT codes on the visit. So say you do an office visit 99214. In the CPT boxes (24d on the 1500 form) you would put 99214 and the other info as needed. Then below that you would put codes for other things, such as pulse oximetry or what-have-you then after the last code, add in an extra code such as G8443 on a new line, just as if it were another procedure.
