A provider just came and informed me that on one patient the diagnosis codes are entered but then when signing the chart he gets the error about no diagnosis code. The codes are in the Assessment field, just like normal, but if he signs it anyway, the codes are not available in the drop down boxes of the Account tab or in the fields there. Anyone else seen this issue or have an idea?

When we made the switch to Version 4 a lot of the diagnosis codes are some how corrupted. For example a 401.1 Benign HTN will not drop into the Assessment field from the patients list of prior DX. If you enter 401.1 in the search engine and try to enter it, it will state, 'the patient already has 401.1 are you sure you want to add it again?" The answer is YES and "401.1 Benign ESSENTIAL HTN" will appear as the assessment. If you do not DELETE the old one from the summary sheet, and you try to use it again in the future, it will not load.

I somehow got the idea this might in some way be related to the problem you described. If it is not, I am sorry I wasted so much of everyone's time.

I think this is a version 4 bug. It was noted in these March 2008 posts.

I certainly have had this happen ( am on version 4.0.60). It happens infrequently and when I had it happen on Wednesday I was certain that it was not a mistake on my part.The diagnosis code was clearly in the assessment box but was not recognized by the CPT box when I tried to sign the chart...I got the same error message.I then erased the DX and pulled it in as a new DX from the coding box,erased and tried to get it recognized by pulling it from the drop down box, but it was not recognized as present when I tried to sign the note. I finally was able to sign the chart by adding a dx code for a minor problem that I would not normally have added. But, the primary diagnosis code did not show up on the superbill. I called support and they did not seem to know what I was talking about or recognize that others had seen the same glitch. The diagnosis code is one that I use frequently, it was frustrating.

I do not understand the exact problem and I hope that it is something that can be corrected. I have tried the same things you mentioned, dklehmannmd, with the same results. It seems to pertain to a patient more than a code for me, as I can use the same codes on another patient with success but on a few select patients the problem occurs.

I have seen that if the colon at the end of the dx is deleted, it will not be recognized as a dx. It must be exactly as it initially came. I wonder if it could have been somehow corrupted and not be in the right format to be recognized. Perhaps reentring it from the lookup would correct the problem?

I agree, it is patient specific. If (when) it happens again I will double check the punctuation.I did try to re-enter the DX from different places, I would think this would bypass the punctuation possibility....if I see it again and figure something out will post back.
