Has anyone else encountered a problem with AC saving patient notes either in another patient's chart or having notes "go missing"? I have a few instances where I write a note and save the note in the patient's record, only to have it show up later under another patient's name. In one instance, there are 2 identical notes under 2 patients' records. Certainly a concern as there is no way to "erase" notes once they are in the record. Any advice or experience with this?

I have had great problems reproducing this, but you can occasionally do this with scrips - from main menu if you have left clicked and highlighted name and then choose diffent name with right click it seems to go the one that was highlighted blue. Does not seem to happen if you click the right chart first so it is highlighted and then right click.... just a thought.
