I am working on an Excell flow sheet to use on some patients where we are auditing the charts and where we feel it can be useful for sorting out the lipids and so forth. My goal is 5 fold. First I want to have a flow sheet for my own use. This would be mainly to review the old history of lipids vs treatments. Second I want to look at Diabetics and detect progression of disease. Third I want to track Diabetics treatments and see that they are getting all the referrals they need. Fourth I want credit for all this work so if someone comes to audit my charts they will be able to find what I have done. Fifth I want it to be easy enough to use that I might actually still be using it in a year.
I have settled on using the flow chart that Lumetra wants you to use, as it is comprehensive and logical. I added one or two things to help me use the same flow sheet for other issues such as renal disease.
I haven't added Peak flow or PFT's but expect to have a space for that also. Why not?
My question for the forum is what graph functions are worth pursuing? HgBA1C of course but what about HgBA1C vs medications used or lipids vs the drug Rx'ed. What are you guys doing? Anyone have a favorite flow sheet?

Hi Doc, I use a workbook with 5 sheets, first page, general stuff, paps, mammos, colonoscopy, bse, tse, etc page 2 lipids, lfts, cardiac crp page 3 diabetes a1c, FSBS, diabetic foot exam page 4 chronic kidney disease-put my proteinuria screens here page 5 INR if necessary
I have been doing this for a month of so now and love it. We are slowly getting all the older data into the flowsheet.
I did have everything on one page, but it was too cumbersome. I'm not thrilled about have so many different pages, but it makes it manageable for me.

Yes! The INR, I forgot that, and that is a flow sheet item that begs for something snazzy that will show the dose and do the math. Does your flow sheet calculate anything like the daily average dose or even fancier does it suggest a new dose by the amount the INR is off target?
