Our docs are having trouble with incomplete notes being deleted when they try to open a second chart. There is no warning; it just deletes the note on the first chart that was open. Is there a patch to this? We just started with AC, so we are likely running the most up to date version. This appears to be a serious flaw in the program; we are loosing visit documentation without knowing it.

Oh my goodness -

This is a major issue for me as well! It's been a problem since the beginning. Just today I lost a note that was nearly completed.

I would love this to be fixed.

with beta version 3.6.2, it gives a warning box saying 'all info will be lost on xyz pt if you switch to another charts' (paraphrased).

Yes, there is a warning, but not always. Just yesterday, I got interrupted while working on a chart, had to close it to get to antoher persons chart and all the new data that had been entered was lost.There was no warning (perhaps a beta issue?).


We had a real problem with one of our laptops - it would just shut down - and of course, if there was an open chart with new data...all was lost. Same thing when I lost power (during storms) a few months ago - all new data that was being entered was lost.

I'm not sure what it would take to have all data saved even if the chart is closed prior to being saved. The practice I worked in previously used Soapware and data was automatically saved if the chart was closed. You only needed to open it back up and continue working.

As an imperfect salvage, don't forget last note saved. It resides in the computer that had the crash or failure. "DON'T" chart anything else on the computer that you were charting that note on until you copy the note you want to retreive or it will be lost and replaced by your new charting!!! You can copy and paste it into the chart of the pt in question....It does help.

or you can also forward the note in progress to your inbox, but that won't help with unexpected shutdowns

Thanks...I've used the backup note and I forward notes frequently...but as I said, its not good enough. When we have power outages, or if one gets distracted (sometimes everything is happen all at once from all directions), I won't remmeber I'm even in a note. I know I can't be the only one this happens to.

Hey wait a minute. I seem to remember somewhere in the early 3. something versions, when we used to get more error message and AC would shut itself down a neat little utility would come up after we would re-open AC. It says something like, "you seemed to have lost the last note you were charting, would you like to resurrect it?" cool How sweet that was the first time we saw it. It was then that I first started considering putting Jon's kids in my will...man if that thing hasn't saved our butts a number of times. Hasen't that little thing come to your rescue more than once in these situations? It's great. Really, we thank Jon out loud everytime that thing kicks in....

A nice enhancement would be to have AC keep all new pt records in a temp file, perhaps labeled with name and date-time, each record held until the corresponding note was forwarded, signed or saved (a new feature we're told is in the works), or the user cleaned the temp folder. Should be relatively easy (easy for me to say...) to implement as an extension of the current "last note" utility.

Clem Surak: Is anyone else using ACPM and the EOBs are not matching up to patient accounts and the EOBs are having to be retyped into ACPM? Started a couple of weeks ago. Payer IDs are correct and match the EOB payer IDs.
tcosta: @ Clem Surak: yes, this has been going on for MONTHS in our office. We've been told it's the payers fault and it has to do with loop segments and payer response. Doesn't make sense to me, considering it's ALL the time now on multiple EOB's a week.
tcosta: It almost never used to happen and now it's constant.
tcosta: ACPM is extremely short staffed and it's taking them days to respond and fix the issue.
Clem Surak: Yes, it has been getting progressively worse. I have a ticket logged, but have not heard back, so I keep retyping the EOBs so that our accounting is up to date. Hope the migration to the new Clearinghouse fixes this issue. Thanks for the response.
