The first website has a telephone number for insurance inquiries, that may be the ultimate answer.

If they want you to use their product, they should help you bill for it. Now as for the product, it would depend on whether you are purchasing it and getting reimbursed (like vaccine) or it is billed direct to insurance (like synagis). Hopefully the latter because most of these small drugs are insanely expensive. Not worth playing the middle man
