Sai asked me about this, and I am sorry to say that I never replied. The reason, though is that I do not know of a good way to do this. There is a long (and sad) history of failed attempts to make this process efficient.
What we want, ideally, is a way for a patient to complete both demographics and a questionnaire of clinical information, and have that seamlessly imported into AC so it need not be re-entered.
Again, ideally, this could be done in advance online (by computer or cellphone) or even in the office on a device before the patient is seen.
The need for an advance, online option is heightened tremendously by Covid and the desire to see patients by telemedicine.
Long-timers here will remember companies like Phreesia, Practicom and others that provided some of this... Indy spent some time on an interface... Updox has some functionality in this area (you can create a rather primitive questionnaire on their portal, but it does not interface well with with AC).
Now Updox has a more expensive option in response to the telemed boom and I am not sure it is much better. If anyone is using it and wants to share their experience, I am all ears.
We have about 2 pages of demos and billing info, plus a 2 page medical questionnaire that we get from every new patient. Now, we email or text it to the patient. Often they are reduced to printing the forms, completing them, and taking a photo on their phone, then emailing or texting it back to us. We import to chart AND staff enters all the demos, etc. Very cumbersome and time consuming.
We have a website. We have toyed with creating fillable pdf's, posting on the site, then having patients complete, download, and send to us. Not sure many of our patients are tech savvy enough to do this.
If anyone has good ideas about how to streamline the process, we would love to hear them.
