We take the position that the "medical record" only includes that part of the record that we generated.
We send all our office notes and about a year of lab and x-ray that WE ordered.
I don't appreciate receiving hundreds of useless PDF's from other doctors, with ER reports, old labs, etc., and we don't trouble anyone with ours.
