Dragon 13 Premium (not Medical) works extremely well in a remote window (Windows RDP) if you have a fairly decent internet connection. And it is easy to build a vocabulary of medical words and terms. You can dictate directly into AC fields.
You can't use it for dictation into AC on the same machine, and it won't work in this window (the AC user board) or UpDox windows. Nuance is very clever at keeping the price of medical dictation high.

Google is great for ordinary speech; only so-so for medical terminology.

Also, I have discovered that V13 works amazingly well with the microphone built into laptops and tablets, so you don't need to use your cell phone, and you actually have a keyboard for corrections -- although Dragon is close to 100% accurate with a little practice.
