If you don't accept insurance right now, I wouldn't worry about the payer id. It's only needed for electronic claims. When you do get eclaims in the future, the payer id will probably only matter at the clearinghouse level, and they will help you get all the info anyway.
The payer id in my billing program doesn't matter. It's the name of the insurance that matches what the clearninghouse has on record and the payer id is assigned there.
