The way I read Side Notes is it is $170 per work group which can be up to 200 users but I could be wrong.
I doubt Google Chat would pass the hospital's strict interpretation of HIPPA.

I'd suggest that you look at all of the Jabber [XMPP] clients that are out there - they make clients that also extend the platform to Android/iOS. The HIPAA police may not want the mobile component.
IF they pay your way to San Diego, I'm willing to give them some time to start the implementation.
