Most EMR's -and EPIC is a prime example - are not designed for the purpose of medical record keeping. They are designed to maximize billing and to maximize the ability to standardize and allow the extraction of data for reporting.
These functions of an EMR are not congruent with (and sometimes are in direct opposition to) our goal which is to keep a record that is readable to us and other docs.
