In the newest versions of AC you have a lot of flexibility in choosing what is included in your CCD - the latest plan is an option to include. This is in 6.5.5 and above.
Actually, you can only include the plan from the most recent encounter, and it's not included in the encounters section of the CCD. If I have referred a patient to another provider and am providing my last three months of notes, it's important that provider be able to see all of each encounter, not limited documentation.
