I think you could give them the HFCA (CMS-1500) form with box 13 left blank and box 27 checked "NO". Or you could mail it in for them to get that fee.
Box 13 assigns benefits to the provider, and normally ours says "Signature on File". Box 27 accepts assignment which would be checked NO since you are not a participating provider.
