There is a nice complete 6 page pdf chart form (with patient handout re: personalized primary prevention) made by the AAFP in the most recent issue of Family Practice Management... can also access in the FPM "toolbox" under the Medicare section... have to be AAFP member to view these tools, though.
One could convert this into a template for AC, or just import the form annually.
