Oh! The form contains all the required elements of the Annual Wellness Visit: Other docs patient is seeing, BP, weight, height, BMI, vision, depression screener, prevention plan, smoking cessation counseling.
AND - as a side note, if patient asks for med refills or you address a problem during exam, you can bill for office E&M code in addition to the G-code which covers the prevention (and other required stuff) only.
