Alas, I have to confess that it is rare I get the chart done in the room. First of all, I have a hard time seeing the small print (over 40 eyes) and secondly, as long as I am there, the patients (who always have several significant oh by the ways - which require at least a few more questions to determine just how significant it is.) keep talking. I have to admit I'm a bit of a perfectionist, since I know I'm not going to remember seeing the person the next day, I need to make sure everything I think is important is in that note.

And yes I have dragon...but I'm not sure it's any better - after several months of use, I should not have to go back and made the same voice corrections over and over again. If I don't catch something before the note is saved...some charts end up sounding comical. frown

I've got to find a better way of doing this. I'd be happy to have them done by 7pm the same day.
