I have one that is an excell spread sheet and the first page is just a list to log all the metrics the people audit for diabetics care. I don't regularly use it, but anticipate using it for an argument during an audit. The second page I use sometime, for coumadin and it is handy as it totals the weekly dose and shows the result divided by seven to let you see at a glance the average daily dose you are using with all of those 1/2 pills and skipped days. I can e-mail it to anyone who wants it. You save it as a blank master in the imported items and then open it in the patients chart, (with excell, you have to have excell which comes with office windows.) and when you save the modified one in the patients chart, you still have the blank master.
