When we made the switch to Version 4 a lot of the diagnosis codes are some how corrupted. For example a 401.1 Benign HTN will not drop into the Assessment field from the patients list of prior DX. If you enter 401.1 in the search engine and try to enter it, it will state, 'the patient already has 401.1 are you sure you want to add it again?" The answer is YES and "401.1 Benign ESSENTIAL HTN" will appear as the assessment. If you do not DELETE the old one from the summary sheet, and you try to use it again in the future, it will not load.
I somehow got the idea this might in some way be related to the problem you described. If it is not, I am sorry I wasted so much of everyone's time.
