I recently did a web conference through Amazing Charts about this company

http://amazingcharts.com/services/partner-community/welltrackone/

They offer eligibility checking, promotion, scheduling, and software to help complete the task the the Medicare Annual Wellness Visit in exchange for a per patient fee. No upfront fee. No annual fee. No Contract.

Anybody have any experience with this service?

Sounds kind of neat, but I don't understand what it accomplishes. Do you still perform the AWV, capture all the Dx codes and, for e.g. HEDIS CPT codes (for IPAs), then upload it to WellTrackONE, then they give you a wellness report of what you just did to begin a f/u care plan?

It seems to be adding an extra step that is unnecessary. Or am I missing the point?

Supposedly they WellTrackONE does a better job than the average office of actually getting the visits completed. I know my office has a low ( maybe 5% ?) rate.

The software seems more intuitive and fast/tablet based rather than a paper template.

The software is designed to be used by a nurse or MA (medicare allows this). So the doctor doesn't see the patient for the AWV (instead the doctor sees patient at followup visit to the AWV, this extra followup visit should more than pay for WellTrackONE's fee I would think [while being beneficial to the patient]).

We've had problems with patients thinking the AWV was a physical. We've also had problems with patients trying to address their chronic medical issues during the AWV.

Some of these problems could be solved without WellTrackONE, I just don't want to expend the energy to re-invent the wheel.

I know these AWV must be a money maker, some division of Lifeline screening is now going out and poaching patients from my practice for a AWV.

How sad.
The whole point of the AWV is to get Medicare patients in to a primary physician, and make it worthwhile for the physician to make the effort.

Lifeline doesn't do any follow up care, and at least in our area, they do scare people into coming in to see me-- requiring duplication of tests and imaging.

Makes money for someone. Not for me.

I wanted to refresh this thread.
Yes, Lifeline is having patients complete forms and see a NP, then billing Medicare for AWV.
Since it can only be done annually, they make it impossible for me to perform this service.
I think it is completely inappropriate.
The AWV should be done (and billed) by the patient's primary care physician.

If this bothers you too, I suggest add a post to their Facebook page, or email them, or Twitter....

How do they get away with that?
I guess there isn't any government program that can't be gamed.
Is lifeline following up by making referrals and providing immunizations, getting home health and Senior Services involved when they spot unsafe situations?
Probably not.
Just another parasite.
Our entire economy is parasites on parasites.

Well, Lifeline deleted my post to their Facebook page!
Reading some of the patient comments there, it seems as if they are just doing this without fully explaining it to them.

We had 2 patients come in for visits after a LIFELINE "screening". My doc was furious when she found out that they had their AWV's with Lifeline. The patients were actually duped into having the AWVs. They went for one thing and ended up having the AWV. My doc found out it was sponsored by a local hospital. She called them and they investigated. Needless to say they are no longer sponsoring Lifeline events.

Exactly what happened to my patient, she went for the vascular screening and lo and behold, I get a report where Lifeline did an AWV.
Needless to say, I will not longer be recommending them.
