Hi everyone. I am a family physician about to launch a solo practice. I have a lot of experience with one of the larger EMR systems, that I was using in my prior job (employed physician in a large group), but I am leaning toward AC for my new, small practice. What I've seen so far is very good. I have one big question, though... There are many situations when I would rather have a flow-sheet type of documentation instead of a separate soap note that is encounter specific. OB is the best example (thus the title of the topic)--normally the entire pregnancy is viewed as an event, rather than the individual prenatal visits. In the paper world, it was the ACOG forms. The EMR I used previously had an OB module, which kept all of the information from the pregnancy in one "place" and allowed a complete prenatal record of the documentation from the entire pregnancy to be easily printed. There are many other applications for this type of "flow-sheet" that is not encounter-specific: health-maintenance, developmental documentation for kids, chronic disease (e.g. diabetes) management. If there is a way to do this type of documentation in AC, I will probably be sold. I think that OB is best example--if there is something people are using for that, I can probably adapt it to my other needs. Thanks, and I'm looking forward to the wisdom of the group!

You can do it in 2 ways
1. Make a template in WORD of the ACOG form and store it outside AC on a shared drive. When the patient has delivered the template can be converted to a PDF file and imported to the pateint's chart in AC
2. Use the ACOG form and use a free PDF editor for each visit and IMPORT it to the patient chart in AC.
Hope this helps

This subject came up a few months back and someone on the boards mentioned [IIRC] doing the ACOG form as an Excel spreadsheet that they imported into AC, and then edited the imported document each time they saw the patient.

I haven't tried migrating any OB stuff into AC. I haven't come up with a great solution yet. I imagine we'll probably do it in editable PDF's when the time comes. I hadn't thought of the Excel spreadsheet.

Does anyone have an excel spreadsheet for OB already made?

I keep Diabetes flowsheets as Excel files stored under "imported items."
We keep a master template in the AC folder called "Templates."
When I have a new diabetic patient, I import the diabetes flowsheet from the templates folder. Customize the form for the patient. Done. Each time we go in and add the new A1C value, document a sensory exam, ask if diabetes eyecare is being done, add the dates of the prior microalbumin/LDL/Cr.

One could easily do this for the ACOG form.
