If anyone has recommendations would love to get them--
We are having problems with disappearing meds on the active/inactive med list; if I look at the note or even the assessment/plan summaries in past visits then I know I've prescribed a med...but when I go to the med list sometimes the medications are mysteriously missing!

We actually had a staff meeting to make sure that no one was deleting medications when they were forwarding charts, but no one admitted to this...

Wondering if there is something different we need to do.
