I have recently created some Online Forms for patients to Fill before their visit to our office - as we are using Telehealth. Here is the link my office send to the patient via text message and they get to Updox Portal to fill in the forms. You all are very welcome to see it how it looks like from patient's view point. Very quick to fill in and submit.
