I do a lot of repeat procedures.
To streamline the paperwork, I fill out the procedure forms.
On the procedure form, there’s a table of things and little checkboxes next to them.
There’re also a few free-entry fields for text input.

I would fill out the forms instead of doing the traditional SOAP notes.
The form was originally created in MS Word, but I can convert it to a PDF file.
