This first screen shot is displaying an icon in the Physical exam which opens a new form to document Functional and Mental Status.



This form allows you to enter text or you will be able to access a template to document. This form is similar to the exiting Cognitive/Functional status form currently on the Summary sheet. Once you click save, we will save the information to the database so it can be incorporated into a transition of care document and we can add the text to the physical exam.
