I have noticed that the ICD 9 code 272.4 (hyperlipidemia) is not being correctly changed to ICD 10 code E78.3. The problem list remains HYPERLIPIDEMIA but the code is (000). This is a very common code in my patients, and will be a burden to fix all of these.
