Most of the health maintenance items are picked up from what you enter in DECISION SUPPORT. These must be entered while you are doing the note, before signing off. Things like demographics and vital signs are picked up from their respective boxes in AC. Then, you must attach a CPT code to the visit when signing off for AC to count it.
You can click the teal MU box at the top of the current note to see if it is picking up your entries before signing off.
This is how it works in V6. I don't know whether it is different in V7.
