We started to do the Medicare AWV(annual wellness visit) few months ago. we have been able to definitely bill for evaluation and management (E&M)(99212-99215) with the modifier 25 and the wellness visit on the same day. As you, we use the AC templates for most the info that goes with the AWV in the same E&M note. Since most our patients are not tech savvy, we do everything when the patient arrives.
A few comments / questions:
I have not found that end of life discussions are part of the AWV. Where did you find this?
I have to say that some of the CMS requirements of the evaluation are vague and can be done in different ways.
For instance, I am assuming that the up and go test is part of your review of functional ability.
What do you use for detection of cognitive impairment ? We use the mini-Cog.
What do you use for screening of status of Depression? We use the PHQ-2 followed by the PHQ-9 if necessary.
