In version 6.0 there was a bug that if you turned on "Time Stamp Chart Fields" it would cause duplication in the Social History field. To fix this in version 6.0 you will have to turn off Time Stamp Chart Fields. To do this click on "Edit" - "My Preferences" - "User Preferences" - "Documentation Preferences". Then uncheck the box for Time Stamp Chart Fields. Or you can upgrade to version 6.3.3, the bug has been fixed in the newer version. Thank you, Anthony@AC
