As I'm sure you know, the problem with the addendum idea is that an addendum appears at the END of the note. So you still don't know that the note is bad unless you read it all the way through to the end. If you start with a previous note as a template, as I do, this means you may unknowingly perpetuate the data from that single bad note, forward indefinitely. Now we can really talk about compromising the integrity of the medical record!
