No reason to have separate charts. Just make sure that the visits are not co-mingled If they are there for a Workmen's Comp. visit, do not cover non-Workmen's Comp. issues and vice versa. We also make sure that we put their claim number at the top of the chart notes so it is clear what the visit is about.
