I also have the same problem - had a 4 page note with multiple duplications of the social hx. I just worked with tech support - it is a bug with time stamps and how many times the chart is forwarded. The number of forwards (desk to nurse to doctor) seems to determine how many repetitions there are. For now the solution is to turn off the time stamp.
