I do the same: My MA sends me a message with the chart attached. I respond to the message. After calling the patient or sending back to the MA or the receptionist, the message is 'SAVE(d) TO CHART'. I only use addendums when addending a chart note.

Remember, if it wasn't documented, it didn't happen.
