I understand what you are looking for, and I also understand your desire not to pay a monthly fee for a document management program. I felt the same way.
On the other hand, I suggest that you consider Updox. It does the things you are looking for, and a lot more. In my opinion, the amount of time and irritation saved by you and your staff would make the program well worth the investment.
