You can do it in various ways. The 2 ways that I do it is
1. Make your form using WORD and save it as a TEMPLATE(Allergy testing form) on the server as David has suggested.
2. You can scan the form as a PDF file and save it. You will need a program like Adobe Acrobat 9 pro which has a TYPEWRITER function form the drop down TOOL menu or whatever program others use on this board to type on a scanned form
You can then retrieve either form and complete it for each visit and save it as the patient's name file.
You can then import it in AC if you wish
