I have added these orders in the lab section:
FASTING BLOOD WORK - NOTHING TO EAT AFTER YOUR EVENING MEAL and
PLEASE COMPLETE THIS BLOODWORK ONE WEEK PRIOR TO YOUR NEXT OFFICE VISIT.
I start each with a leading space to keep them at the top of the list. A quick pick when needed.

I have also added the CPT code to the order name for common labs so it will print on the order sheet. I've had no problems with laboratories accepting my printed orders, and no confusion.
