I was wrong yesterday, the comment does not get attatched to the imported item no matter which way it is signed off. I am interested in: not getting sued, not settling a suit and reduced malpractice premiums. To the end I want a medical record that supports me . While I am complaining, the date that the item was signed off is not noted and there is no equivalent of my initials on the document either. At my office I am the only one who can sign off, so by deduction I must have seen it. But, an outsider reviewing the record would not be able to tell that.
