If you use the TWAIN scanning interface in AC you can only use the pre set categories. However, at any time you may right click on a file, select edit, then give the file an arbitrary category. After you have done that you can drag new files to that category.
You may instead opt to scan to a folder then drag the file from that folder into the AC imported items window. It will default to 'unspecified' name. Then you can rename and categorize the files as you wish.

I suspect this area will be improved after another version plus a couple sub versions.
