Instant Medical History is used by some AC chart users. It is an algorithm-based tool to gather information but is no substitution for taking the information yourself or having a trusted aide do it for you. It relies on discreet data (yes, no, sometimes, rarely...) and assumes some medical sophistication on the part of the patient (not to mention computer literacy, and well, plain old literacy - which is a stretch for some of my patients in a bilingual rural community). It also further distances the practitioner and staff from the patient, in my opinion. I am not a fan, but I encourage you to try for yourself and decide for yourself. Check out the website at
