What are we expected to do when we close a note thinking everything has been addressed, then the patient (or patients parent) adds something at the last moment? How can one edit the previous note without creating a new one? Or, are providers expected to create a second note on top of the first?

You would have to add a second note. Alternatively, you could either do an addendum or a message. I generally just add it in a message and then in the subject line write something like Addendum to note 9/2/08.

The addendum will let you add a billing charge, although I don't use that anyway.
