We seem to have charts "go missing" at least a few times each week. There are people that are on the schedule, we remember seeing them, they even have a diagnosis in the summary area for that day and yet the note is just gone..poof.. Anyone else having this problem? Something else I need to be doing? Something I am doing wrong?
